Standard Cleaning Plan
- Two prophylaxis cleanings
- Two exams with Dr. Goebbel
- Two sets of X-rays
- Two fluoride varnish treatments
- Oral cancer screening with cleanings
- 22% discount on all other services, not including products
$180 down w/$35 month (until $349 is reached) or $349 per year
Periodontal Maintenance Plan
- Three periodontal maintenance cleanings
- Two exams with Dr. Goebbel
- Two sets of x-rays with exams
- 3 Fluoride varnish treatments
- Oral cancer screening with cleanings
- 22% discount on all other services(not including products)
$200 down w/$50 month (Until $499 is reached) or $499 per year
Each plan is valid for a year from the purchase date. If payment is not made on time, a two-week grace period will be granted first occurrence. After second occurrence, they will lose the right to do monthly payments and will need to do yearly fee. All payments must be up to date before your appointment will be scheduled.
This is a Dental Service Club Membership, not a dental insurance. It is a program for prepaid dental services offered by Dr. Goebbel. This membership cannot be in combination with any insurance or discounts.
